With reference to above, we are pleased to place this order for supply of  "Inj Piperacillin (powder for inj. 4gm) + Tazobactum (500mg)" as per “DETAILS OF ITEMS & PRICES” and “TERMS & CONDITIONS” stated herein below:

Terms & Conditions of the Supply order is described below: -

  • DELIVERY PERIOD : Supply of minimum 30% of the ordered quantitity to be complated within 10th days from the date of placement of order. Another 30% of the ordered quantitity to be complated within 15th days from the date of placement of order and last 40% of the ordered quantitity to be complated within 25th days from the date of placement of order. But every efforts should be made to complate the supply within best possible time in view of medical emmergency (specially for containment of COVID-19 pandemic).

 

  • The order stands cancelled after the expiration of delivery period mentioned above plus additional 7 days to complate the delivery as stated in the tender, and if the extension is not granted by the TIA. Once the supply order will be cancelled, the firm will be liable for debarment for the particular product for a period of 6 months in NHM, Assam.

 

  • TEST REPORT: Supplier must submit Batch wise Test Analysis report (in original) from NABL Certified Labs for every batch of items on or before commencement of supply. Minimum in house report of the batch to be supplied is to be enclosed along with challan during the time of delivery. But the Batch wise Test Analysis report (in original) from NABL Certified Labs is to be submitted along with bill to be claim on completion of order.
