With reference to above, we are pleased to place this emergency purchase order for supply of Crash Cart Trolley, Bedside Screen and IV Stand as per “DETAILS OF ITEMS & PRICES” and “TERMS & CONDITIONS” stated herein below:

Terms & Conditions of the Supply order is described below: -
- The above price is on FOR destination basis inclusive of all expenses including transportation, transit insurance, loading &unloading, installation and any other incidental charge.
- The supplied items shall be strictly in conformity with the specifications of the tender. If the items are found to be inferior or substandard in quality the same must be replaced with acceptable quality at your cost within a period of two weeks from date of intimation. In case of failure to replace the defective goods on time or in the event the replaced goods are found inferior or substandard in quality, you will be liable to be blacklisted.
- Supply should be completed within 7 days due to emergency requirement for containment of COVID 19. However, you are requested to complete the supply & installation earlier.
- In case there is delay in delivery beyond the stipulated period as indicated in the delivery schedule, there shall be reduction in price @ 0.5% of the value of delayed goods per week of delay or part thereof subject to a maximum of 10% of the total order value.
- Payment terms will be 100 % within 30 days from date of receipt and acceptance. Full payment shall be made after successful completion of supply and installation of the entire ordered quantity and against submission of bills and receipted challans bearing stock entry details.
- Within 5 days of issue of Purchase Order, you shall be required to submit Security Deposit of 5 % of the order value in the form of bank guarantee from any Indian nationalized bank valid for a period of one year from the date of the order in favour of “State Health Society, Assam”. The BG is to be renewed prior to its expiry for a period of another one year to cover the full warranty period. However, if you fail to execute the order or to perform the services as per agreement, in addition to other penal actions, the bank guarantee shall be encashed and the amount forfeited.
- All other Terms & Conditions incorporated in the tender referred to above shall be applicable.
