With reference to above, we are pleased to place this order for supply of Injection Enoxaparin 40 mg/0.4 ml(PFS) as per “DETAILS OF ITEMS & PRICES” and “TERMS & CONDITIONS” stated herein below:

Terms & Conditions of the Supply order is described below: -

  • 5,000 numbers of Injection to be supplied by 28th September 2020, another 50,000 numbers of Injections to be supplied by 29th October 2020, another 50,000 Injections to be supplied by 12th November 2020 & supply of remaining 50,000 numbers of injections to be completed by 25th November 2020. However due to COVID-19 pandemic, you are hereby requested kindly to complete the supply within the best possible time.

  • Payment will be made within best possible time from receipt and acceptance of full quantities of ordered materials at destination against submission of bill & receipted challan (in triplicate) with stock entry details.
  • If a supplier fails to execute successive order within the stipulated time frame, in full, there will be an additional time of 7 days and after that the supply order will stand cancelled and that firm will be liable to debarred for the particular product, for a period of 6 months in NHM, Assam tender.
  • Rates of Drugs should not be more than rates approved by GOI(NPPA). If rates are more than NPPA approved rates, then excess amount will be recovered from the manufacturer.
  • Any Dispute arising out of the above order shall be referred to the Court within the Guahati Jurisdiction of State of Assam.
  • All other Terms & Conditions as specified in the TENDER NO:  NHM-18013/2/2020-PROC-NHM/ECF-145501/23-P, Date:-08.09.2020 shall be applicable.
