Sir,

With reference to above and in view of the urgent need of various essential and life saving items for public health care service due to global outbreak of COVID 19, you are requested to supply the following item as per the given  terms and conditions:

Terms & Conditions of the Supply order is described below: -

  1. The above price is on FOR destination basis inclusive of all costs including GST, transportation, transit insurance, loading & unloading and any other incidental charge. All admissible taxes as applicable as per existing Govt. rules are liable to be deducted from invoices.
  2. Supply Should be completed within 10 days from the date of issue of this P.O
  3. In case there is delay in delivery beyond the stipulated period as mentioned, there shall be reduction in price @0.5% of the value of delayed goods per week of delay or part thereof subject to a maximum of 10% of the total order value.
  4. The suppliers shall have to enter into a "Deed of Agreement" for execution of the supply order as per terms & condition given herewith within 10(ten) days of issue of this order.
  5. The Supplier shall have to furnish “Security Deposit” @ 5% of the total order value in the form of “Bank Guarantee” of a scheduled Indian Bank in favour of “State Health Society, Assam” valid for a period of 1 year from the date of the order.
  6. Eighty (80) % payment will be released on delivery of items  and balance Twenty  (20) % will be released after satisfactory installation of the items.
  7. All other Terms & Conditions as specified in the tender reference Tender No.NHM-41011/6/2018-IDSP-NHM/92-P, Date: 25/04/2020 shall be applicable.
