Terms & Conditions of the Supply order is described below: -

 

  • Full ordered quantity of N-95 Mask and Triple Layer Mask must be delivered by 15 days at NHM, SHQ, Store,Narengi,Guwahati from the date of issue of the purchase order.  
  • Manufacturer must submit original copy of batch wise Test Analysis report from NABL Certified Labs/NIB/CDL(Lab must have sope for testing of N-95 and Triple Layer Mask) for every batch of items before commencement of supply. The bidder shall supply the goods to the designated warehouses, only after receipt of "Despatch Clearance Certificate" from the Tender Inviting Authority/Procuring Agency. 
  • If a supplier fails to execute successive order within the stipulated time frame, in full. In case there is delay in delivery beyond the stipulated period as mentioned in the purchase order, there shall be penal deduction @ 1% of the value of delayed goods per week of delay or part thereof subject to a maximum of 5% of the total order value. Once the maximum price reduction is reached, termination of the contract may be considered. Non-performance shall make the successful bidder liable to be disqualified to participate in any tender for the next 3 years for that product, in addition to forfeiture of Security Deposit and other penal actions.
  • The successful bidder shall submit Security Deposit of 5 % of the order value in the form of Bank Guarantee/ Demand Draft/Fixed Deposit from any Indian Schedule Bank in favour of “State Health Society, Assam” within 5-7 working days from date of issuing of Purchase Order by Tender Inviting Authority. Such Security Deposit shall be valid for a period of 3 months from the date submission of the Security Deposit.
  • Payment will be made within 30 days from receipt and acceptance of materials at destination against submission of bill & receipted challan (in triplicate) with stock entry details to obtain the desired payment against the supply from the NHM-Authority.
  • Any Dispute arising out of the above order shall be referred to the Court within the Guahati Jurisdiction of State of Assam.
  • All other Terms & Conditions incorporated in the NHM-18012/1/2020-PROC-NHM/ECF-132136/5-P, Date:-30.01.2020. shall be applicable.
