With reference to above, we are pleased to place this order for supply of PPE Kit as per “DETAILS OF ITEMS & PRICES” and “TERMS & CONDITIONS” stated herein below:

Terms & Conditions of the Supply order is described below: -

  • 10% ordered quantity(500 Kit) to be delivered within 3 days from placement of order and and rest 90% (4500 Kit) quantity of ordered item to be delivered by next 7 days at NHM, SHQ, Store, Narengi, Guwahati.
  • Payment will be made within 30 days from receipt and acceptance of materials at destination against submission of bill & receipted challan (in triplicate) with stock entry details to obtain the desired payment against the supply from the NHM-Authority.
  • Any Dispute arising out of the above order shall be referred to the Court within the Guahati Jurisdiction of State of Assam.
