With reference to above, we are pleased to place this order for Printing & Supply of  Handbook for Screening Visible Birth defects at all ASHA as per “DETAILS OF ITEMS & PRICES” and “TERMS & CONDITIONS” stated herein below:

Terms & Conditions of the Supply order is described below: -

A Bank Guarantee of Rs. 31,000/- (Rupees Thirty  thousand only) for the Item  having validity not less than one year as security deposit is to be submitted within 7 days from the date of order placed.

Delivery: The supply order should be delivered within 30 days from the date of purchase order. The item should be delivered at NHM District warehouse.

Penalty:

1. In case there is delay in delivery beyond the stipulated period as mentioned in the purchase order, there shall be penal deduction @ 0.5 per cent of the value    of the delayed goods per week of delay or part thereof subject to a maximum of 10 per cent of the total order value.
2. Once the maximum price reduction is reached, termination of the contract may be considered. Non-compliance of the contract provisions shall make the successful  bidder liable to be disqualified to participate in any tender for the next 5 years, in addition to forfeiture of Security Deposit and other penal actions.

All other Terms & Conditions as specified in the TENDER NO: NHM/CH/IYCF/Flipchart/2014-15/2324/Pt-I/23229, DATE:-18-11-2017 shall be applicable.
